Called via traverse_enums.  Emit an enum definition for
   enum_type *SLOT.

References enum_value::def, md_constant::name, enum_type::name, enum_value::next, enum_type::num_values, upcase_string(), md_constant::value, and enum_type::values.

   minimize dependencies.
